“Frailty” is a term clinicians use to describe vulnerable older Australians, though it is not always easy to measure. The Edmonton Frailty Scale is a reliable tool for assessing frailty and will measure a consumer’s frailty, identify specific elements of frailty that warrant a follow-up, and inform care planning and appropriate support.
